• Follow a link to our Patient Portal, a separate website operated by our electronic medical records provider (polarisemr.com).

This website itself does not provide a patient portal, live chat, or any way to receive medical advice or treatment directly through the site. Submitting a form does not create a doctor-patient relationship and does not guarantee that an appointment will be scheduled.

Not for Medical Emergencies
