• Appointment request form: whether you are a new or existing patient; patient name and date of birth; guarantor name and date of birth (if different from the patient); your email address and phone number(s); preferred and secondary appointment date/time; insurance name, policy number, and group number; referring physician; and any additional comments you include.
• Provider referral form (available to referring providers on our Provider Forms page): patient name, date of birth, gender, and phone number; insurance and subscriber information; the referring provider's and clinic's name, phone, and fax; diagnosis and ICD-10 code; special instructions; and, optionally, uploaded files such as a copy of the patient's insurance card or recent clinic notes and diagnostic results (PDF, JPG, or PNG).

Our Employment page does not collect information through this website — it invites applicants to email their materials directly to our office, so nothing you send there passes through this website's forms.

We do not use cookies, analytics tools, or advertising/tracking scripts on this website. We do not collect information about your browsing activity beyond what you voluntarily submit through the forms above.

How We Use Your Information

Information submitted through our contact, appointment request, and provider referral forms is sent by email to the appropriate CCM Specialty Care staff (general inquiries, scheduling, or referral coordination, respectively) so that we can respond to your message, appointment request, or referral. We use this information only for the purpose you submitted it for.

These forms are delivered using a third-party transactional email service (SendGrid). Any files you upload to the provider referral form are attached directly to that email and are not separately stored on our servers or in a database.

How We Share Your Information

We do not sell your personal information. We do not share the information you submit through our website forms with third parties for their own marketing purposes. Your information is only shared internally with CCM Specialty Care staff involved in responding to your inquiry, appointment request, or referral, or with the third-party email delivery provider described above, strictly to the extent needed to deliver that email.

Mobile opt-in, SMS consent, and phone numbers collected for SMS communication purposes will not be shared with any third party or affiliates for marketing purposes.

Patient Portal

This website links to a separate, third-party Patient Portal (polarisemr.com) operated by our electronic medical records provider. That portal has its own privacy practices, which are governed by its own privacy policy, not this one. We encourage you to review it separately.

Health Information and Security

Our provider referral form and appointment request form can involve health-related information, such as a diagnosis, insurance details, or clinical documents. This website does not represent that it is HIPAA-certified or that a signed Business Associate Agreement is in place for these forms; we are stating this plainly rather than implying a certification or safeguard that is not in place. Detailed medical history and clinical records are otherwise collected and handled through our in-office intake process and the separate Patient Portal referenced above, not through this website's public forms.

Data Retention

We retain information submitted through our website forms only as long as necessary to respond to your request or appointment, or as required by applicable law.

Children's Privacy

Our practice provides pediatric care, and appointment or contact forms are generally submitted by a parent or guardian on behalf of a minor patient. We do not knowingly collect personal information directly from children through this website outside of that context.
